Thanks Gliwmaeden2. I got past the initial Qs (twice) but it timed out saying the agents were busy. Pretty frustrating. Hopefully they will see this and email me the form, it blatantly ought to be available for anyone to download. Guessing this sort of stuff explains the poor customer service ratings.

The trouble with the new Chat system is we seem to have to get past the initial questions before being put through to a real person. The range of topics is very limited - they really need at least to have "other" as an option, and I will feed this back.
It may be that all the agents are engaged, @Ben__L. Live Chat should be available at this time of day on a Sunday.
