Change of account ownership form

Hi, I have tried phoning three times, emailed & written to Talk Talk to get a change of ownership form but have not received one? What do I need to do to get one? My initial correspondance was answered informing that I had downloaded & had sent the wrong forms & the correct form would be sent, that didn`t happen either.

It is important that the account ownership is changed, thank you.

If you would like us to look into this, please can you update your community profile to include your home telephone number, we will then be able to email you the correct form.

To do this go into 'my settings' then 'personal information' then add your details.
