I received a letter and Application form from TalkTalk saying "You have recently informed us that you would like to receive our Advance Care Service." but I didn't inform them about this in fact I didn't know that such a service existed.
Anyone else received one or know anything about it?

Well I would feel a bit of a fraud if I allowed it to remain.
Any explanation as to how it came to be flagged?

This was changed on the 8th Jan. Did you not requested this?

Talktalk do priority fault repairs for people dependent on their landline to support healthcare etc, @Stoker.
