Nominated user form

I need a nominated user form please.  This is so i can manage my mothers account.  She is hard of hearing and cant discuss this her self on the phone, she gets confused and bit scared.

I can look into this but first, can you please update your community profile to include:

Name

Telephone number or account number.

You'll need to complete your community forum profile details, @nanoitch, but there may be further security checks needed too.

Staff will reply when they reach your thread in the queue. 

Please add your Talktalk landline number to your community forum profile details.  Go via your avatar/name; settings; from the drop down menu that starts with "email", choose Personal Information. All your own details go in here.

At the end of that section, in Private Notes, add your mother's details. 

SAVE CHANGES.
