Top Cashback Claim order number and gift voucher

Hi, I signed up to TalkTalk via TCB 2 months ago using the Cyber Monday compare broadband offer for £92 cashback and a £150 gift voucher.

 

My purchase hasn't tracked for the gift voucher or  TCB (I did take a screenshot) and I need the order number to send to Top Cashback. Can you provide it please as I don't have an order number in any of the TalkTalk emails. Also when will the £150 gift voucher be sent?
