I have asked TalkTalk four times now for a return kit for my old router, and the only one they sent had a faulty label. I'm worried they will charge me for a late return as it's been over a month and they still haven't sent anything. Anyone else having this issue?

They had a bad complete batch of returns labels, so, yes, many people had this problem.
You should have automatically been sent a replacement, valid label, @mirandakcross.
