My modem is dead

Woken up this morning, no lights at all on the modem. Tried restart, tried different plug socket, the cable has gone. Using a neighbours old one now and that works but how do I get hold of someone to order a replacement?

Is this the TalkTalk router or Openreach modem?

 

Please can you update your Community Profile to include your name and TalkTalk landline number.
